Fetterman has frequently diverged from mainstream Democratic positions on foreign and domestic policy. He has voiced support for U.S.-Israeli military action against Iran, voted to advance Senator Markwayne Mullin's nomination for Homeland Security Secretary, and cast a decisive vote against measures to block a potential military invasion of Cuba. He has also stated that access to Trump's White House ballroom is essential, praised ICE operations at airports as an "enhancement," and criticized the majority of Democrats and Democratic-leaning independents for holding negative views of Israel.

During a Fox News appearance in late April, Fetterman was asked about his standing within his party. He laughed and raised his hands before characterizing his relationship with fellow Democrats as "cordial." He noted his surprise at being unpopular within the party, remarking that he is a Democrat who flipped a competitive Senate seat.
